Sexual Violence Notification Form

The Notification process does NOT initiate a complaint or investigative process. It is a mechanism to facilitate easy referrals for students to access timely support and to help connect staff and faculty with SVPRS for consultation. We suggest that you complete the notification form with the student when that is possible. Identifying information about the student should not be included without consent.  If a student name is included, a member of the SVPRS team will reach out to offer support and services. The form should be completed for every disclosure.  We always confirm receipt of the notification form with the author.

Section 8 of the policy states that employees who receive a disclosure of sexual violence from a student must notify SVPR Services.  Additionally, when a disclosure is made, employees must provide the student with information about the policy and available supports.  When making a notification, information that identifies the student should only be shared with the student's consent. This notification/referral process may also be used for disclosures of intimate partner violence. 

 

Responding to Disclosures for Staff and Faculty

In this in-person workshop, staff and faculty will learn how to respond empathetically to disclosures of sexual violence and understand the Queen's policy and their professional responsibility. Workshops can be requested by contacting SVPREducation@queensu.ca
